The Social Security Administration released an updated Form SSA-89 Authorization for the Social Security Administration (SSA) to Release Social Security Number (SSN).
Brokers and NDCs should being using the updated Form SSA-89, dated 12-2020 effective immediately as the SSA will no longer accept Form SSA-89 dated 2-2018.
The updated Form SSA-89, pre-populated with Homebridge information, has been posted on the Homebridge website at www.HomebridgeWholesale.com Forms page under General Forms,
