Homebridge has made updates to two form requirements on the Fannie Mae HomeStyle, and FHA 203(k) Standard and Limited renovation programs.
Contractor Profile Report (Fannie Mae Form 1202)
The Contractor Profile Report has been replaced with the Contractor Questionnaire – Full Form and the Contractor Questionnaire – Limited Form.
- The Contractor Questionnaire – Full Form is for use on Fannie Mae HomeStyle and the FHA 203(k) Standard programs when the renovation project is > $35,000
- The Contractor Questionnaire – Limited Form is for use on Fannie Mae HomeStyle and the FHA 203(k) Limited programs when the renovation project is ≤ $35,000
Permit Certification/Acknowledgment Form
The Permit/Certification/Acknowledgment form has been updated to include plumbing as a permit type. As a reminder, this form is required on all renovation transactions.
The new and updated forms are required for new submissions on or after June 13, 2023.
